Pre-Release Notification for Committed Youth

Upon receipt of a Pre-Release Notification (PRN) form from commitment programs, the JPO/contract case manager notifies the school of the youth's anticipated release.

Long Term Commitment Youth:

Thirty (30) days prior to the youth's projected release from the commitment program, the JPO/contact case manager notifies the School District of Lee County of the youth's anticipated release., A Pre-Release Notification (PRN) form, and Individual Educational Plan (IEP if the student was identified as needed Exceptional Student Education services), the name of the JPO, should be mailed or faxed to the following address:

Short Term Commitment Youth:

For programs with lengths of stay of 45 days or less, the JPO/contract case manager notifies the School District of Lee County of the youth's anticipated release. The written notification should be faxed to:

Report to register and be assigned to a District school. Upon the youth's return to the community (Lee County), he/she should report with his/her parent/guardian to the nearest Parent Information Center/School CHOICE Office (school system) within 24 hours of the youth's release to enroll and be assigned a school. The youth should bring new proof of residence with them. Indicated below are the following two locations for the School CHOICE offices:
